Current and former DART employees who did not agree to receive an ACA 1095-C Form electronically may request that a copy of the form be mailed to them. Requests can be made by:
- Sending an email to HR@ridedart.com.
- Mailing a written request to Attn: Human Resources, 1100 DART Way, Des Moines, IA 50309.
- Calling 515-283-8100 to reach the administrative office.
Please confirm your mailing address in the request. You will receive the form in the mail within 30 days of the request.
